Please complete ALL 3 steps:

1.

Complete the Preparticipation Physical Evaluation form (Both pages)

• Medical Eligibility Form

• • MUST be completed by a licensed MD, DO or Surgeon
  • MUST have physician signature AND office stamp
  • Kaiser physical forms are accepted

2.

Go to https://athleticclearance.com/

• Complete ALL 5 sections

• • Medical Insurance coverage is REQUIRED
  • If needed, information is available in the Front Office or go to https://myers-stevens.com/enrollment-page/

• For those students without an ID number, please enter  0000
• Choose “Troy” from the list and “2025-2026” as the school year

• Once completed, please print the “Confirmation Page” and sign

3.

Scan both the Medical Eligibility Form (1 Page) AND the SIGNED Confirmation Page to the website. (Please see below)
